Veterinary Services & Animal Welfare (Farming, Animals & Conservation) Full time The role We are seeking a Care Clinician to join our team. Reporting to a Clinical Lead, and part of a growing team, this role acts as a clinical resource to the Support at Home Services Team (HCP) and provides clinical input to client assessment and care planning, which may include client clinical assessments, identifying client clinical risks, developing client care plans, and supporting to initiate referrals where required. What you will do Conduct comprehensive and accurate nursing assessments using appropriate assessment tools. Implement evidence-based nursing interventions and treatments based on assessment findings and client goals. Support the development of comprehensive care planning documentation and/or recommendations, including a timeframe based on assessed needs. Coordinate with health care professionals to ensure integrated and holistic care delivery. Monitor client's health, progress, response to treatments, adjusting care plans as necessary to optimise positive outcomes. Educate clients and families on health conditions, treatment options and self-care techniques. Provide skilled nursing care, including but not limited to personal care, wound management, palliative and mental health care support. Maintain accurate and detailed clinical documentation and records in compliance with regulatory standards and organisational policies. Collaborate with Care Advisors to manage complex clinical needs and support decision-making in line with the clients' preferences. Undertake all duties in a manner with due regard for the wellbeing and safety of self, peers and care recipients in accordance with workplace health and safety policies and procedures. What we look for Division 1 Registered Nurse with current AHPRA registration. Proven experience in clinical care management, preferably within the home care sector. A strong commitment to excellence and quality in the provision of aged care related services with a sound working knowledge of quality principles and accreditation. Dedication to delivering high-quality, patient-centred care while maintaining a focus on duty of care obligations. Proficiency in Greek language is desirable. Valid driver's licence, access to your own vehicle and maintain comprehensive insurance coverage with a willingness to travel across metropolitan Melbourne, as required. Why choose Fronditha Care?
