The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.

Based on-site within residential aged care facilities across Victoria, you will facilitate clinical management and coordination efforts in this key role.
You will orchestrate seamless rounds with Geriatricians, Physicians, and GPs, meticulously coordinating appointments, and fostering clear communication between on-site staff and remote healthcare providers.

Proficiency in computer usage is a must, and while prior telehealth experience is preferred, it is not mandatory.

Essential Criteria Bachelor's or Diploma of Nursing.
Experience in problem-solving and professional collaboration.

AHPRA Nursing Registration.
