Based on-site within residential aged care facilities across Victoria, you will facilitate clinical management and coordination efforts in this key role. Your role will involve orchestrating seamless rounds with Geriatricians, Physicians, and GPs, meticulously coordinating appointments, and fostering clear communication between on-site staff and remote healthcare providers. The Registered or Enrolled Nurse will collaborate within a multi-disciplinary team, ensuring efficient and high-quality service delivery, backed by the support of the Nursing Regional Manager and the administration team. The successful candidate will contribute to our care model by engaging in organic business development to expand the resident base. Proficiency in computer usage is a must, and while prior telehealth experience is preferred, it is not mandatory. Key Responsibilities: Facilitate clinical management and coordination efforts within aged care facilities. Orchestrate rounds with Geriatricians, Physicians, and GPs. Coordinate appointments and communicate effectively with on-site staff and remote healthcare providers. Engage in organic business development to expand the resident base. Provide high-quality service delivery as part of a multi-disciplinary team. Minimum Requirements: Bachelor's or Diploma of Nursing. AHPRA Nursing Registration. Current driver's license and reliable vehicle. Experience in problem-solving and professional collaboration. IT skills and proficiency in documenting customer needs. Aged Care experience, leadership preferred. Skills & Experience Experience in similar roles ensuring individual health needs are met. Organization and time management skills. Relationship-building and excellent communication skills. Proficiency in computer usage. Aged Care experience and understanding of clinical scope. Knowledge of person-centered care.
