If you're looking to be refreshed in your nursing experience, we may just be what you are looking for... As the Clinical Care Coordinator, you will need: Up-to-date knowledge of aged care including Clinical Governance.
Leadership that models empowerment to residents and staff through sharing information.
Excellent communication, interpersonal, and customer service skills, with the ability to foster collaboration with residents and staff.
Organisational and problem-solving skills, with the ability to manage multiple tasks and priorities.
Demonstrated ability and willingness to work in a team and independently.
Demonstrated effective interpersonal and communication skills (written and verbal).
Minimum three (3) years' experience in residential aged care.
Sound understanding of care assessment in the aged care sector.
Main Duties of the Role: Manage all clinical services in line with our process and policy.
Advocate the implementation of continuous improvement activities in all aspects of clinical and lifestyle services.
Ensure all clinical documentation is completed accurately and on time.
Coordinate and support all scheduled audits and clinical indicators completed in line with the schedule.
Ensure resident health records and clinical documentation is relevant and up to date.
Ensure "best practice" medication management guidelines are observed and regularly monitored.
The successful candidate will: Contribute to the efficient and effective management of human, material and fiscal resource management according to the strategic plan and budget in line with the Mission and Philosophy of Seventh-day Adventist Aged Care (Victoria) Ltd.
Provide overall clinical monitoring demonstrated in a way that reflects Case Management.
Maintain timely and accurate care planning, evaluations and care consultations as allocated.
Supervise, support and encourage staff to perform their roles and responsibilities in accordance with duties lists and residents care plans.
Be responsible for maintaining the continuity of care by follow up communication from previous shift and undertaking regular checks of the facility.
Ensure rights and responsibilities of residents are provided and follow up concerns raised by residents or their families.
Coordinate continuous improvement activities relevant to resident care.
Provide human resource management including recruitment, selection, appointment, rostering, staff development and performance management.
Maintain the role as the IPC lead and be responsible for the infection control protocol in the facility.
You Must have the following: A current and valid police check (no more than three months old) in the absence of a NDIS clearance.
