About the role Adult 'step up, step down' Prevention and Recovery Care (APARC) is a short stay residential service, delivered in partnership with Victorian hospital networks and Health Service, supporting clients aged 25-65 years with psychosocial disability to transition from an acute hospital setting or prevent hospital admissions for those entering directly from the community.
The service provides individualised 1:1 support, shared supports, and group work based on client's strengths to enable people to achieve their recovery goals, enhance relationships with families and carers, develop social skills, build community networks, and increase confidence and capacity to live safely in the community.
We are seeking compassionate, self-motivated Community Mental Health Practitioner – Relief Workers to work alongside a caring team on a 24/7 rotating roster in Bendigo.
You will provide intensive person-centred, recovery-oriented supports to clients to achieve their wellbeing goals and build capacity to live independently in the community.
Key responsibilities Deliver high quality services and facilitate positive educational and social experiences.
Support clients to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
Provide information and mentoring to clients to establish relationships with families and carers, create community networks, and access relevant services.
Plan and deliver group programs that aid clients to build their skills and focus on recovery.
Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client's recovery goals.
What you'll bring Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
Expertise in working with people with mental health issues, complex needs, and interacting with families and carers.
Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma-informed care, and family inclusive practice.
Relevant experience in facilitating group activities for clients experiencing mental ill health.
Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams, and key stakeholders.
