About the Opportunity The Ord Valley Aboriginal Health Service now has an opportunity for an NDIS Community Connector Coordinator (CCC) to join their team of dedicated health professionals in Kununurra, WA on a full-time basis.
Reporting to the NDIS Manager, you will be primarily responsible for: Facilitating NDIS Scheme eligibility testing, access and coordination of plan; Acting as a key point of contact for potential participants and families, and other key stakeholders; Ensuring the focus of care is on the needs of Aboriginal patients within the context of their cultural identity, family and social circumstances.

About You OVAHS is seeking an experienced individual with a background in healthcare or social service delivery, and a thorough understanding of healthcare providers.
Due to the nature of this role, well-developed report writing skills will be essential.
A strong understanding of the impact of disability on daily life and how reasonable and necessary supports can impact on a person's ability to participate in community and economic life will also be crucial for your success.
While you will be very well supported by the OVAHS team, you will be living and working in a remote part of Australia; therefore you will need to be resilient and adaptable, with good communication skills and the ability to build trust within the local communities in which OVAHS operates.
Experience with stakeholder engagement will ensure your ability to liaise effectively with both patients and their families, service providers, and pathway supports.
Importantly, you will be committed to providing culturally sensitive healthcare and demonstrate an interest in working closely with Aboriginal communities to understand their health issues and work with them to find solutions.
An appropriate level of empathy will enhance your ability to act as an advocate for Aboriginal patients, doing your best to ensure individuals and families receive the care they need.
