About the role The Nurse Practitioner (HARP) is a registered nurse who is authorised to function autonomously and collaboratively in an advanced and expanded role.
The Nurse Practitioner role includes assessment and management of consumers using nursing knowledge and skills which may include, but not limited to, the direct referral of consumers to other health care professionals, prescribing medications and ordering diagnostic interventions.
The Nurse Practitioner will have strong knowledge of working in the community setting with family inclusive practice a core element of their care.
The Nurse Practitioner role is grounded in the nursing professional values, knowledge, theories, practice and provides innovative and flexible health care delivery that compliments other health care providers.
The scope of practice is determined by the context in which the Nurse Practitioner is authorised to practice, according to the Nursing & Midwifery Board of Australia (NMBA) competency and accreditation standards and is articulated in a specific model of care.
In HARP this is a social model of care focusing on self-management and education to prevent the readmission into hospital.
HARP services aim to prevent avoidable hospital presentations by implementing time limited care for clients with complex or chronic conditions who frequently attend hospitals or are at imminent risk of Hospitalisation.
The HARP Nurse Practitioner will be actively involved with all HARP clients, using their credentialing and specialised practice to support clients at home within this multi-disciplinary team.
About you To be successful in this role you will possess: Registered Nurse with the Nursing & Midwifery Board of Australia (NMBA), with no conditions on registration Completed approved master's degree of Nurse Practitioner or equivalent Clinical Master's degree, as deemed appropriate by Nursing and Midwifery Board of Australia via AHPRA, including the Therapeutic Medication management module.
Endorsement as a Nurse Practitioner via the NMBA.
Current Drivers Licence Desirable Research publications and presentations at conferences.
5 years post graduate experience Demonstrated skills in Chronic Disease management in the community setting.
Previous experience working with psychosocial needs of patients.
Expertise in access management around patient flow, triage, and clinical prioritisation practices in line with the DHHS Access policy.
Previous exposure to working with KPIs.
Excellent communication skills, written, verbal and interpersonal with demonstrated ability to collaborate and work as an effective team member at a local and organisational level to deliver organisational outcomes.
Demonstrated negotiation, problem solving and analytical skills.
Membership of relevant specialty Professional Association.
Vaccination against infectious disease is a mandatory requirement of this role.
