Job Summary: Provide client-centered assessments and interventions to promote independence and safety in the home and community. Collaborate with a multidisciplinary team to address complex physical and psychosocial needs. Support clients through comprehensive care planning, equipment prescription, and home modifications. Job Description: This role offers the opportunity to provide high-quality occupational therapy services to clients under community-based programs. As an Occupational Therapist, you will conduct comprehensive assessments, create tailored care plans, and support clients to live independently in their homes. The position involves collaborating with multidisciplinary teams, prescribing assistive equipment, recommending home modifications, and engaging in health promotion activities. You will work across various sites to deliver interventions and ensure client safety and well-being. Key Duties: Conduct comprehensive physical and psychosocial assessments to identify client needs. Develop and implement individualized care plans focused on maximizing client independence. Recommend and prescribe assistive equipment and home modifications, following Australian Building Standards. Provide episodic care, manage referrals, and support chronic disease groups. Maintain accurate records, including digital medical records, funding applications, and client progress reports. Participate in quality improvement initiatives and data collection to enhance service delivery. Supervise other clinicians, Allied Health Assistants, and students as needed. Collaborate with other health professionals to ensure a holistic approach to client care. Skills and Experience: Current registration with the Occupational Therapy Board of Australia (AHPRA). Strong understanding of client-centered care, self-management principles, and reablement models. Experience working in community settings, conducting home assessments, and prescribing equipment. Excellent communication, problem-solving, and time management skills. Ability to work independently in the community and as part of a multidisciplinary team. Victorian driver's license is required.
