About HIP: The Health Independence Program (HIP) is made up of a number of programs including community rehabilitation, specialist services and clinics, and care management.
The Fast Track Care Management Team is a highly capable and energetic multidisciplinary team of nursing and allied health professionals with support from geriatricians that provides short term care management for vulnerable clients in the community.

We offer an exciting opportunity for an Occupational Therapist to work with a great bunch of people supporting a diverse range of clients.
The Occupational Therapist provides important advice, clinical assessment and support for our clients so they are able to remain safely at home.
About the role: This position is part time (3 days/24 hours a week) fixed term until 31/12/2024 and is a great opportunity to try a new role with a new team where you will make a real difference to the lives of our clients.
As the Fast Track Occupational Therapist you will: be based mainly at the Repatriation campus of Austin Hospital, but will collaborate across Austin Health work with clients and families in their homes work with clients and families in collaboration with the care managers to provide a safe home environment for clients provide advice about occupational therapy across the fast track and chronic and complex client teams be supported by the broader HIP community to enhance your own learning and development A regular day will see you: applying your skills, knowledge and creative thinking to develop client centred care plans and working with clients to achieve their goals working both on campus and in the community having fun working with an experienced multidisciplinary team Key responsibilities: client-centred assessment, care planning and client management of community-based clients provide reports, ongoing referrals and recommendations for longer term management, equipment and home modifications contribute positively to the multidisciplinary team environment About you: You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care.
You'll bring energy and drive to your work, and have a keen interest in developing your skillset.
