About the opportunity The Person-Centred Practice Specialist plays a pivotal role in enhancing person-centred practice, care and support across Bolton Clarke Residential Homes.
Reporting to the Clinical Practice and Projects Manager, this position focuses on co-designing, implementing, and supporting person-centred learning implementing best practice strategies, particularly for residents with complex needs such as dementia.
The role involves collaborating with internal teams and external partners to develop best practices, deliver training, and foster a culture that prioritizes resident well-being, safety, and dignity.
Through expertise, coaching, and continuous improvement, the Specialist ensures that every resident experiences a fulfilling and person-centred quality of life.
Your day-to-day responsibilities will include: Person Centred Learning and Development: Co-design, plan, and implement person-centred learning strategies, programs, and priorities in collaboration with internal and external stakeholders to enhance the skills and knowledge of the workforce.
Support and Expertise in Person Centred Care: Provide expertise for complex person-centred care needs, support critical incident responses, and assist in developing improvement plans to enhance resident outcomes.
Coaching and Capacity Building: Use coaching methodologies to build person-centred practice knowledge, skills, and practice among home leaders, promoting safety and well-being for residents.
Data Analysis and Continuous Improvement: Analyse data to identify improvement opportunities, report on person-centred outcomes, and participate in policy and procedure development to enhance care practices.
Policy and Procedure Development: Contribute to the development and review of policies, procedures, and frameworks that support and guide person-centred practices across the organization.
Leadership and Collaboration: Lead and collaborate in person-centred practice delivery, ensuring sustainable approaches through a network of facilitators and coaches within homes and support teams.
Role Modelling and Advocacy: Consistently role model person-centred values and practices, promoting a culture of respect, dignity, and empowerment for residents, families, and team members.
Support for Operational Teams: Act as a subject matter expert for operations, homes, and central support teams to guide the delivery of person-centred care, including participation in continuous improvement initiatives and compliance with relevant standards.
We would love to hear from you if: You are a highly skilled professional with expertise in Person Centred Dementia Studies and extensive experience in person-centred practice within residential aged care settings.
You excel at co-designing and implementing learning initiatives, demonstrating strong leadership, coaching abilities, and effective communication and stakeholder management skills.
Committed to continuous improvement and with a deep understanding of dementia care, you are dedicated to enhancing resident outcomes and fostering a person-centred culture across the organization.
Skills / Qualifications: Postgraduate qualification in Person Centred Dementia Studies or related aged care qualification is desirable.
Expertise in person-centred practice, especially in dementia care within a residential aged care setting.
Proven experience in co-designing and implementing person-centred learning and development initiatives.
Strong leadership, facilitation, coaching, and influencing skills to drive practice improvements.
Ability to analyse data to support continuous improvement and evidence-based care approaches.
Excellent communication and stakeholder management skills to collaborate effectively with diverse teams and partners.
