• Liaising with medical and allied health professionals and clients' family members • Supervising and supporting our Personal Care Workers.
• Assessment, planning, documentation and evaluation • Implement the intervention/s identified in the plan of care for each Clients and ensure these directives are evaluated, communicated and documented • Identify Client care requirements by establishing personal rapport with participants • Maintain safe and clean working environment by complying with procedures, rules, and regulations; and calling for assistance from health care support personnel • Maintain Client's confidence and protects operations by keeping information confidential.
