The Brimbank Melton Children's Health and Wellbeing Local will provide holistic supports to infants, children and families, delivering services like psychology and psychiatry, allied health, family supports and paediatricians.
People with lived experience will also form an important part of the Local's workforce.
The service will support children aged 0-11 who are experiencing developmental, behavioural or emotional challenges and their families, and will focus on early intervention and providing age-appropriate care.
Within this rewarding role, as Family Support Practitioner you will be part of a multidisciplinary team to deliver effective, collaborative, family-centred services.
Based at IPC Health Sunshine campus location, the Local will move to a purpose facility in Caroline Springs in the future.
This position is a Full Time ongoing role and is classified as level 5 under the Social and Community Services award.
The pay point will be determined by the successful candidates level of experience.
The position is classified as a Social and Community Services Employee Level 5 under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022.
You will make a difference by Empowering clients to recognise their strengths, aspirations and wellbeing goals and facilitate supportive connections that will help them achieve these.
Providing secondary consultations to Family services and Universal services.
Building the capacity of families to respond to their child's needs through coaching, mentoring and information sharing.
Make timely referrals to appropriate support and specialist services identified in the care plan.
Develop and implement a shared and coordinated approach that incorporates social, emotional and physical determinants of health through collaboration.
Contributing to the improvement of the Local's service by informing the stand up and service design aligned to key performance indicators.
Contributing to program content and activities which support the wellbeing of infants, children and families.
To succeed you will need: Qualification in Social Work or relevant field Minimum 4 years relevant experience working in partnership with families who have children aged 0-11 years.
Excellent motivational interviewing and holistic wellbeing assessment skills.
Strong communication with the ability to liaise with a wide range of stakeholders to create and maintain effective partnerships which generate improved outcomes for families.
