About the opportunity Reporting directly to the Chief Operating Officer, this newly created role as the Occupational Therapist (OT) will be responsible for conducing OT specific assessments, interventions and planning within the NDIS and Aged Care frameworks, to increase peoples function and independence in all aspects of their lives.
The role will provide high quality, effective and efficient service to clients with a focus on providing person centred, holistic and goal directed care that promotes clients' wellness and active participation in decisions about their care.
A day in the life as the OT with Care Choice, some of your key tasks include: Provide evidence based, person-centred assessment, clinical management and intervention to people with a cognitive, physical or psycho-social disability in line with NDIS goals.
Prescribe Assistive Technology and recommendations for Home Modifications funded under the NDIS and other external funding agencies, this includes the preparation of funding applications.
Provide ADL (activity of daily living) and functional capacity assessments within the client's home and provide recommendation and intervention.
Manage a caseload of clients who may present with increasing complexity, multiple co-morbities and functional limitations across multiple areas of their life.
Utilise a strengths-based, client centred approach to support a client to identify their goals and utilise a capacity building approach towards achievement and long term sustainability of these goals.
Provide information and skills training to clients, their family and support workers as needed to achieve the NDIS goals.
Liaise with and provide therapy and progress reports to the NDIS, Support Coordination, external organisations and other referrers/funding bodies as required.
Participate in quality improvement and allied health service delivery projects and meetings as directed.
Write reports to a high standard that meet both the needs of the client and the insurer.
Develop and maintain an up to date and in depth understanding of NDIS legislation, rules and regulations and how this applies to the delivery of community based allied health services and other capacity building supports under the NDIS.
Participate in supervision and annual performance appraisals.
Ensure compliance with all relevant legislation Contribute to the development of the internal and client communication plan.
Lead delivery of internal communication.

Ideally, you'll have: Strong knowledge and understanding of the NDIS Prior work experience or lived experience of disability Demonstrated experience in person centred care through effective assessment, care planning, case conferencing and review of clients Demonstrable anatomical and neurological expertise to understand the physical difficulties clients are experiencing A commitment to Care Choice values, vision and mission, and ability to engage in the enhancement of Care Choice culture in meeting organisational objectives Highly developed written and verbal communication skills Proven track record in creating efficiency and meeting targets Knowledge and experience in assistive technology & home modifications Empathy and the ability to build rapport Excellent investigative skills and the ability to problem solve and develop solutions Ability to work as part of a multidisciplinary team Qualifications required Tertiary qualification in Occupational Therapy Current AHPRA Registration Current First Aid & CPR Certificate NDIS Worker Screening Clearance Working with Children Check (Employee type) Valid Victorian Driver's Licence with access to a comprehensively insured vehicle To APPLY If this sounds like the ideal role for you and you have the skills and experience we're looking for, we'd love to hear from you.
