We have an exciting Senior (Grade 3) opportunity for an experienced Allied Health (PT/OT/SW) clinician to work in the ED Care Coordination Team at Western Health in a 0.9 capacity until 9 July 2025. The ED Care Coordination team is part of the Western Health at Home Division based at our Footscray, Sunshine and Williamstown EDs. The team's role is to assist with coordinated discharge planning for patients with complex care requirements, helping to get them home sooner and connecting them with community support services.
